Now a safety speaker working out of Perth I had capped a rewarding career of 14 years with a leading multinational industrial company with production and safety milestone achievements and a global award for a project success. And then……….disaster struck.

While trying to start a similar project to one that had recently gone so well, I was one of a group of four that was severely injured. A fifth team member had a near miss. Hundreds of millions of dollars of equipment was destroyed as a result of the accident. The many earlier successes now all seemed blown away by the searing pain.
Together the team had more than a half a century of experience on similar facilities combining hands-on know-how with supervisory savvy and both technical and managerial competence. They were revered by their colleagues across the globe. So what went so horribly wrong?
